•HSCT for thalassemia is a long-term value-for-money intervention in a developing country.•The clinical and economic benefits of HSCT far outweigh TC.•The ICER (QALY) with HSCT as compared with TC is Image 1664,096 (US$986) in MRD transplantation.•The probability of HSCT to be cost effective at the willingness to pay threshold of Indian GDP per capita is 94%.
