•Fluoxetine reduces the three-year recurrence rate of ischemic stroke significantly.•The main mechanism is inhibiting platelet aggregation at early stage after stroke.•Improving traditional risk factors of stroke and negative mood are other mechanisms.•Prospective randomized controlled study for stroke, whether or not with depression.
