•PED should be recognized as a primary treatment for a highly selected cohort with predominantly complex ICA aneurysms.•Aneurysm complexity and novel BD scale predict occlusion probability and may help determine number of per-aneurysm devices.•We present excellent long-term neurologic and radioanatomic results in patients with predominantly complex ICA aneurysms.
