•Drug-resistant Pseudomonas aeruginosa poses a treatment challenge for intensive care unit (ICU) patients.•Extended infusion (EI) of carbapenem antibiotics has been studied as one of the strategies to tackle this challenge.•Biapenem, a newer carbapenem antibiotic, has shown more potent activity than imipenem or meropenem against P. aeruginosa.•Limited studies have assessed if EI of biapenem can optimize pharmacokinetics/pharmacodynamics index in ICU patients.•Traditional biapenem 0.5 h short infusion regimens were insufficient against P. aeruginosa in ICU patients.
