•A more extended resection (TLR) in the left, dominant, hemisphere causes a worse outcome in verbal memory performance.•Our data does not show a difference in seizure outcome between the TLR and SAH approach.•Long term seizure outcome (seven-years, longitudinal measure), remains favorable in patients with hippocampal sclerosis.•Epilepsy surgery in the temporal lobe leads to worse verbal learning in patients operated in the left, dominant, hemisphere.
