•This part 3 of the consensus report by the Pediatric Acute Lung Injury and Sepsis Investigators and Pediatric Blood and Marrow Transplantation Consortium provides evidence-based guidelines for the supportive care of children with veno-occlusive disease (VOD) who develop multiorgan dysfunction.•The roles of noninvasive ventilation, invasive ventilation, and high-level cardiac output monitoring strategies are discussed.•Infection-related issues in patients with VOD are discussed.•Guidelines are provided for the treatment of fulminant hepatic failure and hepatic encephalopathy.•Strategies to prevent and treat delirium in children with VOD are discussed.
