•An ultrathin choledochoscope and holmium laser lithotripsy (HLL) is effective for treating cholecystocholedocholithiasis.•Laparoscopic transcystic common bile duct exploration (LTCBDE) outcomes are improved with an ultrathin choledochoscope/HLL.•Use of an ultrathin choledochoscope/HLL is associated with minimal complications when performing LTCBDE.
