Impact of errors in paper-based and computerized diabetes management with decision support for hospitalized patients with type 2 diabetes. A post-hoc analysis of a before and after study

•Paper group: manual insulin dose calculations were major source of error.•8 times more BG measurements were influenced by dosing errors.•Dosing errors led to hypoglycemic episodes, OR 3.1 (95% CI: 1.4–6.8).•Computerized group: calculation errors avoided, workflow investigation possible.•Workflow deviations led to hyperglycemic episodes, OR 2.2 (95% CI: 1.1–4.6).
