•Diagnostic imaging plays a major role in patients with skeletal neoplasms.•Diagnostic accuracy of imaging methods in skeletal neoplasms should be proven by pathology as the reference standard.•MRI and FDG-PET/CT showed overall comparable diagnostic accuracy in skeletal neoplasms.•Modality accuracy varied in subgroups consisting of bone matrix response and localization of the primary neoplasm.
