•AGE-IM is a case-control study carried out on an Italian cohort.•Total PUFA, omega-6 and omega-3 PUFA were reduced in MI patients.•MI risk significantly and steadily decreased with increasing levels of PUFA.•Both omega-6 and omega-3 were related to MI risk reduction.
