Clinical results of Obwegeser II method for correction of severe mandibular protrusion

Background and Objectives: Various surgical techniques for correction of mandibular protrusion have been reported. Of these we have mainly selected sagittal split ramus osteotomy (SSRO) with/without maxillary osteotomy for the reason of safety of the surgical procedure and wide indication. However we could not perform SSRO in following cases: 1) severe mandibular protrusion without maxillary hypoplasia; and 2) severe mandibular protrusion with cleft lip and palate (apprehension of velopharyngeal incompetence due to maxillary advancement). Obwegeser reported another technique of intraorally resecting mandibular angle (Obwegeser II method) in 1964. We have used the Obwegeser II method in cases with severe mandibular protrusion. The purpose of this study was to evaluate the postoperative results of Obwegeser II method.
