My research agenda has developed over the past 30 years with a primary interest in the role of the effects of exercise and physical activity on cardiorespiratory function and control. This work is largely interdisciplinary, and has now evolved into both basic and clinical research investigating the interaction of arterial, autonomic and immune function on subclinical atherosclerosis and how these interactions are modulated by nutrition, exercise, and physical activity. I have a specific interest in how exercise training affects autonomic function and how autonomic function impacts cardiovascular fitness in a variety of populations, including, children, obesity, diabetes, Down syndrome, multiple sclerosis, kidney disease and kidney transplant. Over the past 10 years I have also developed an active line of research in physiologic repsonses to firefighting, using live fire scenarios. Recently, I have also started a line of work investigating exercise and health disparities. I have conducted many clinical trials using both endurance and resistance exercise in healthy individuals, populations with cardiovascular disease, old and young (children) individuals, as well as in obese populations with diabetes. A consistent trend in my work over the past 30 years have been investigating cardiovascular fitness and determinants of cardiovascular fitness in individuals with intellectual disabilities, with a focus on individuals with Down syndrome. This work has focused on understanding reasons and mechanisms for the low work capacity in these populations, developing appropriate and valid test procedures for both laboratory and field tests, and understanding the effect of exercise training in the population. We were the first to document that the very low VO2max, work capacity and ability to exercise in persons with Down syndrome are to a large extent due to autonomic dysfunction, primarily due to altered sympathetic function. The impact of this work is evidenced by the fact that the top 3 most cited manuscripts in this area and over half of the top 10 most cited manuscripts in this area are manuscripts from our team.
